On or about 2/28/2018 at a worksite located at 1017 High in Topeka, Kansas, during excavation and utility line installation activities, employees working in an excavation were exposed to struck by hazards from the proximal spoils piles.

29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652 (b) or (c). On or about 2/28/2018 at a worksite located at 1017 High, Topeka, KS: Employees engaged in utility line installation activities, working in a trench found to be approximately 5 and 6 feet in depth, were exposed to struck-by and cave-in hazards in that the trench was not properly benched and alternative protective systems were not used.
