29 CFR 1926.451(c)(1): Supported scaffolds with a height to base width (including outrigger supports, if used) ratio of more than four to one (4:1) were not restrained from tipping by guying, tying, bracing, or equivalent means, as set forth in subparagraphs (i) - (iii) of this paragraph.  a) Jobsite - The supported tubular welded frame scaffold, measuring approximately 32 feet 8 inches in elevation and 5 feet I inch in width, was not restrained from tipping or falling over by guying, tying, bracing, or equivalent means.  Employees were constructing a concrete elevator shaft from the outrigger of the 4th working platform, approximately 26 feet above ground level.  Observed on or about 3/1/18.

29 CFR 1926.451(e)(1): A means of access for scaffold platforms more than 2 feet (0.6 m) above or below a point of access, such as portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from another scaffold, structure, personnel hoist, or similar surface was not used.   a) Jobsite - The employer did not provide a safe means of access from one level of the tubular supported welded frame scaffold to another by the use of portable ladders, hook-on ladders or other acceptable means.  Employees climbed the scaffold supports to access the 4th working levels  Noted on or about 3/1/18.

29 CFR 1926.454(a): The employer did not have each employee who performed work on a scaffold be trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used, and to understand the procedures to control or minimize those hazards.  a) Jobsite: The employer did not train their employees to recognize the hazards that are present when working from supported tubular welded frame scaffolds, and to understand the procedures to control or minimize those hazards.  The employer was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 1180058, citation number 1, item number 1b, and was affirmed as a final order on 2/7/17, with respect to a workplace located at 600 N 3rd Street, Philadelphia, PA.  Noted on or about 3/1/18.
