29 CFR 1910.23(c)(11): Portable ladders used to gain access to an upper landing surface did not have side rails that extended at least 3 feet (0.9 m) above the upper landing surface.    On or about March 1, 2018, the employer did not ensure the ladder side rails extended at least three (3) feet above the landing.  Employees connecting the duct work of an HVAC unit used the ladder to access and exit the roof.  This exposed the employees to fall hazards.

29 CFR 1910.30(b)(1):  The employer did not train each employee in the proper care, inspection, storage, and use of equipment covered by this Subpart before an employee used the equipment.    On or about March 1, 2018, the employer did not ensure employees, who were required to use ladders, had been provided training that would allow the employee to recognize the hazards related to using ladders and the procedures to follow to minimize those hazards.  This exposed the employees to fall hazards.
