Location:   a) Scaffold on the stairway between the fourth floor and the roof:       On or about 02/06/18, two employees were installing sheetrocks while standing on folded A-Frame ladders that was on a scaffold platform supported by a ladder and the step of stair. The scaffold platform was not at least 18 inches wide. Employees are exposed to falling approximately 12 feet to the fourth floor ground level.

The scaffold platform is not supported by stable object. Employees are exposed to falling approximately 12 feet to the fourth floor ground level due to scaffold collapse.
