29 CFR 1926.405(a)(2)(ii)(I): Flexible cords and cables used for temporary wiring were not protected from damage: a) On or about 02/14/18, during construction (renovation) of the At Home Store, located on 3049 West Ridge Road, in Rochester, New York, an extension cord was placed across the entryway used by construction vehicles and employees to gain access to job sites. The cord was exposed to damage from vehicles driving over the cord; mashing the insulating jacket and conductors. NO ABATEMENT CERTIFICATION REQUIRED
