29 CFR 1904.41(a)(2): 29 CFR 1904.41(a)(2) Annual electronic submission of OSHA Form 300A Summary of Work-Related Injuries and Illnesses by establishments with 20 or more employees but fewer than 250 employees in designated industries. If your establishment had 20 or more employees but fewer than 250 employees at any time during the previous calendar year, and your establishment is classified in an industry listed in appendix A to subpart E of this part, then you must electronically submit information from OSHA Form 300A Summary of Work-Related Injuries and Illnesses to OSHA or OSHA's designee. You must submit the information once a year, no later than the date listed in paragraph (c) of this section of the year after the calendar year covered by the form.  402 Fulton St., New Iberia, LA (Fulton Machine Shop, Inc.):  On or about February 27, 2018, the employer failed during calendar year 2016, to electronically submit information from their OSHA Form 300A or equivalent by 12/15/2017. The establishment employed 21 employees and is classified 332710 during calendar year 2017.

29 CFR 1910.179(g)(1)(v): Pendant control box(es) were not clearly marked for identification of functions:    Instance a)  On or about February 27, 2018, manual machines area, the pendant control box on a one ton "Budgit"  overhead crane was missing an up/down push button and the remaining button  was not clearly marked for identification of functions.     Instance b)  On or about February 27, 2018, manual machines area, the up/down push button knobs on a  2 ton overhead crane (model unclear) was not clearly marked for identification of functions.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  Machine Shop:  On or about February 27, 2018, a horizontal bandsaw (HEM-SAW) used to cut metal stock did not have a safety guard on the unused portion of the saw blade.  Approximately 10" of the blade was exposed on the left side of the band saw.
