29 CFR 1926.703(b)(6): Concrete formwork shoring: All base plates, shore heads, extension devices, and adjustment screws shall be in firm contact, and secured when necessary, with the foundation and the form: a) Employees were exposed to struck-by hazard; A 12 ft tall metal shore post fell over, hitting an employee. The threaded collar adjustment was maxed out and the top of the post (shore head) was not nailed to the overhead stringer beam. Location: 1227 Broadway, New York, N.Y. On or about 10/20/2017 NOTE: BECAUSE ABATEMENT OF THIS VIOLATION IS ALREADY DOCUMENTED IN THE CASEFILE, THE EMPLOYER NEED NOT SUBMIT CERTIFICATION NOR DOCUMENTATION OF ABATEMENT OF THIS VIOLATION AS NORMALLY REQUIRED BY 29 CFR 1903.19.
