29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.  a). 2nd Floor Balcony - On or about February 26, 2018, an employee operated a pneumatic Paslode Strip Nailer (model: PF-350S) without the use of eye protection, thus exposing the employee to a struck-by hazard.

29 CFR 1926.102(a)(3): The employer did not ensure that each affected employee who wears prescription lenses while engaged in operations that involve eye hazards wears eye protection that incorporates the prescription in its design, or wears eye protection that can be worn over the prescription lenses without disturbing the proper position of the prescription lenses or the protective lenses.  a). 2nd Floor Balcony - On or about February 26, 2018, an employee wore prescription lenses without the use of appropriate eye protection over his prescription lenses or eye protection that incorporated the prescription in its design when operating a Bostitch pneumatic nail gun, thus exposing employees to a struck-by hazard.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501(b).  a). 2nd Floor Balcony - On or about and times prior to February 26, 2018, employees worked from the balcony's rafters while installing plywood onto the rafters without the use of a fall protection system, thus exposing employees to a fall hazard up to approximately 29.4 feet.  b). 2nd Floor Balcony - On or about and times prior to February 26, 2018, employees worked from the balcony floor deck without the use of a fall protection system, thus exposing employees to a fall hazard of approximately 15.9 feet.
