At the worksite - On or about February 27, 2018, employees operated Hitachi pneumatic nail guns without the use of eye protection, thus exposing employees to a struck-by hazard.

At the worksite - On or about February 27, 2018, employees installed roof shingles onto the building's roof without the use of a fall protection system, thus exposing employees to a fall hazard up to approximately 16.4 feet.

At the worksite - On or about February 27, 2018, the employer did not ensure employees were trained by a competent person in the use of personal fall protection equipment to include, but not limited to the usage of proper anchorage points, thus exposing employees to a fall hazard up to approximately 16.4 feet.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed; or if the ladder's length did not allow the extension, the ladder was not secured at its top to a rigid support that will not deflect and provided with a grasping device such as a grabrail to assist employees in mounting and dismounting the ladder.  a). At the worksite - On or about February 27, 2018, a portable extension ladder did not have its side rails extended at least three (3) feet above the roof's landing surface nor was it secured at the top by a rigid support, thus exposing employees to a fall hazard of approximately 10 feet.
