On or about March 1, 2018, at the front of the Fab Shop, the employer did not ensure that the power strip utilized to provide electricity to the microwave and grinder originated in an approved power outlet.  A multi-outlet extension cord was used to provide power to a six (6) outlet power strip connected in series (daisy-chained).
