Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that caused or were likely to cause death or serious physical harm to employees in that employees were exposed to the hazards of crushing: On or about March 1, 2018 and at times prior thereto, employees were exposed to crushing hazards while operating three (3) forklifts which were not equipped with seatbelts to transport and stage materials; two (2) Datsun Forklifts and a Nissan Forklift (Model Variation CPJO1A18EV).

29 CFR 1910.305(a)(2)(v)(A): Branch circuits did not originate in an approved power outlet or panelboard. On or about March 1, 2018, at the front of the Fab Shop, the employer did not ensure that the power strip utilized to provide electricity to the microwave and grinder originated in an approved power outlet. A multi-outlet extension cord was used to provide power to a six (6) outlet power strip connected in series (daisy-chained).
