On or about 03/03/2018 and times prior, employees were working on the ground in an area where heavy equipment such as, but not limited to, dump trucks and loaders, were operating exposing employees to a struck-by hazard.

29 CFR 1910.253(b)(2)(ii): Inside of buildings, cylinders were not stored in a well-protected, well-ventilated, dry location, at least 20 (6.1 m) feet from highly combustible materials such as oil or excelsior. Assigned storage spaces were not located where cylinders would not be knocked over or damaged by passing or falling objects, or subject to tampering by unauthorized persons:   a. Coastal Landfill maintenance area - Inside of buildings, cylinders were not stored in a well-protected, well-ventilated, dry location, at least 20 (6.1 m) feet from highly combustible materials such as oil or excelsior. The oxygen and acetylene compressed gas cylinders were stored within 20 feet of a 55-gallon drum of used oil and two 5-gallon containers of gasoline.  b. Coastal Landfill maintenance area - Oxygen and acetylene compressed gas cylinders were not secured from falling.   c. Coastal Landfill maintenance area - Oxygen and acetylene compressed gas cylinders were stored together.
