29 CFR 1926.501(b)(13): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems. On or about March 01, 2018, employees were working on the roof of a residential home without using any form of fall protection. Employees were exposed to fall hazards of more than six feet to the ground below.

29 CFR 1926.503(c)(3): The employer did not retrain affected employees who already had been trained but demonstrated inadequate understanding and skill required by paragraph (a) of this section. On or about March 01, 2018, employees were working on the roof of a residential home without having adequate understanding and skill in using fall protection equipment. This condition exposed employees to fall hazards greater than six feet.
