(b)  Block 9 Lot 10 Sky Mesa Subdivision: On and around February 16, 2018 the electrical power cord of a circular saw was damaged. Power cord was connected to the damaged energized extension cord (110 volt).  Okly Construction, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.416(e)(1), which was contained in OSHA inspection number 1236446 citation number 1 item number 2b and was affirmed as a final order on  10/20/17 with respect to a workplace located at 3563 S.
