29 CFR 1910.22(a)(2): The floor of each workroom was not maintained in a clean and, to the extent feasible, in a dry condition.     Food Service / Nutrition Area: Standing water was accumulating on the walking and working surfaces. Floors were not maintained in a dry condition. Employees were exposed to slip and fall hazards.    The U.S. Department of Veterans Affairs was previously cited for a violation of this occupational safety and health standard or its equivalent standard [29 CFR 1910.22(a)(2)], which was contained in OSHA inspection number 1098059, citation number 2, item number 1 and was affirmed as a final order on March 14, 2016, with respect to a workplace located at 4150 Clement Street, San Francisco, California 94121.  The U.S. Department of Veterans Affairs was previously cited for a violation of this occupational safety and health standard or its equivalent standard [29 CFR 1910.22(a)(2)], which was contained in OSHA inspection number 1074778, citation number 2, item number 1 and was affirmed as a final order on August 18, 2015, with respect to a workplace located at 2615 E. 29 CFR 1910.37(b)(4): Signs were not posted along the exit access indicating the direction of travel to the nearest exit and exit discharge when the direction of travel to the exit or exit discharge was not immediately apparent:  In the following locations, signs indicating the direction of travel to the nearest exit were not posted along the exit route. The exit discharges were not apparent throughout the building. Employees were exposed to fire hazards.  a) Warehousing/Receiving Area: Employees had to travel through sliding doors and corridors to reach an exit discharge. b) Hospital Shipping/Receiving Area: The exit discharge was not apparent and employees had to travel through the Material Supply & Distribution Area for the exit discharge.  The U.S. Department of Veterans Affairs was previously cited for a violation of this occupational safety and health standard or its equivalent standard [29 CFR 1910.37(b)(4)], which was contained in OSHA inspection number 1189249, citation number 2, item number 1, and was affirmed as a final order on January 4, 2017, with respect to a workplace located at 4801 Veterans Drive, Saint Cloud, Minnesota 56303.

29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling:  Sub Basement Pipe Shop: Electrical equipment, such as relocatable power taps, were used in a manner that was not approved:  a) The relocatable power tap for the desk/workstation had an electrical cord and another power tap daisy-chained together to energize a floor lamp. b) The relocatable power tap for the desk/workstation was providing power to the microwave.  Employees were exposed to electrical and/or fire hazards.   The U.S. Department of Veterans Affairs was previously cited for a violation of this occupational safety and health standard or its equivalent standard [29 CFR 1910.303(b)(2)] which was contained in OSHA inspection number 1167347, citation number 2, item number 4, and was affirmed as a final order on January 12, 2017 with respect to a workplace located at 11201 Benton Street, Loma Linda, CA 92357.  The U.S. Department of Veterans Affairs was previously cited for a violation of this occupational safety and health standard or its equivalent standard [29 CFR 1910.303(b)(2)] which was contained in OSHA inspection number 1101615, citation number 1, item number 4, and was affirmed as a final order on May 17, 2016 with respect to a workplace located at 381 Miranda Avenue, Palo Alto, CA 94304. 29 CFR 1910.305(g)(1)(iv): Flexible cords and/ or cables were used for purposes prohibited by paragraphs (A) through (F):  Sub Basement Pipe Shop: An orange extension cord was used in a prohibited manner to provide power to a relocatable power tap and floor lamp: a) The extension cord was used in lieu of permanent wiring. b) The extension cord was run through the wall of the Pipe Shop.  Employees were exposed to electrical and/or fire hazards.
