29 CFR 1910.22(a)(2): The floor of each workroom was not maintained in a clean and, to the extent feasible, in a dry condition.    On or about March 2, 2018, employees were exposed to slip and struck-by hazards when water was permitted to puddle on the floor due to roof leaks in areas where employees walk and drive powered industrial trucks.  In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1910.305(b)(1)(i): Conductors entering cutout boxes, cabinets, or fittings were not protected from abrasions:  On or about March 2, 2018, employees were exposed to shock hazards on Ross line 3 and Ross line 4 when the electrical cords from the motors to the power source passed through an opening in the guard without protection from abrasion.  In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  a) On or about March 2, 2018, employees were exposed to caught-in hazards when the rollers at the dip tank on Line 7 in the Ross department were not guarded.  b)  On or about March 2, 2018, machine operators in the Ross Department who operate and monitor the treatment line  were exposed to fracture hazards where the front rollers were not guarded on line 7.  ContiTech USA, Inc. dba Specialty Yarn & Converting, Inc. was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 1099473, citation number 2, item number 1 and was affirmed as a final order on August 16, 2017, with respect to a workplace located at 1 Osmundy Street in Porterdale, GA  30070.

On or about 12/22/2017, material handler  an employee was struck in the face by an air hose resulting in a broken tooth.
