29 CFR 1926.502(d)(17): The attachment point of the body harness was not located in the center of the wearer's back near shoulder level, or above the wearer's head: On or about 2/20/2018 at 1560 South Main Street Greensboro, GA workers were observed performing roofing operation above 25-feet exposed to injuries such as but not limited to broken bones and rips and tears in the skin where the harnesses were not tight-fitting where the "D" ring was not centered in the wearer's back.
