29 CFR 1926.602(c)(1)(ii): Modifications or additions affecting the safe operation of the equipment were made without the manufacturer's written approval: a) Jobsite, Lot 252, Weatherburn Heights, Valencia, PA 16059: On or before February 28, 2018, an employee was using a wooden attachment on the forks of the powered industrial truck to hoist personnel into the air. The equipment's data plate did not reflect the homemade wooden attachment.
