29 CFR 1926.1203(b)(1): If the workplace contains one or more permit spaces, the employer who identifies, or who receives notice of, a permit space must inform exposed employees by posting danger signs or by any other equally effective means, of the existence and location of, and the danger posed by, each permit space.  A sign reading "DANGER-PERMIT-REQUIRED CONFINED SPACE, DO NOT ENTER" or using other similar language would satisfy the requirement for a sign:    On or about 03/05/2018, at 185 Hibiscus Drive, in Miami, Florida, the employer failed to inform employees of a permit confined space by properly identifying a manhole space, exposing employees to a potential of asphyxiation hazard.

29 CFR 1926.1205(a): Before entry was authorized, the employer did not document the completion of measures required by paragraph 1926.1204(c) of this standard by preparing an entry permit:    On or about 03/05/2018, at 185 Hibiscus Drive, Miami, FL 33101, the employer failed to prepare an entry permit prior to allowing a worker from entering a manhole, exposing the worker to an asphyxiation hazard.

29 CFR 1926.1203(e)(2)(ii): When entrance covers were removed the opening was not immediately guarded by a railing, temporary cover, or other temporary barrier that would prevent an accidental fall through the opening and that would protect each employee working in the space from foreign objects entering the space:    On or about 03/05/2018, at 185 Hibiscus Drive, Miami, FL 33101, employees walking and working in the immediate area of a manhole that was not guarded were exposed to a fall hazard of approximately 9 feet.
