On or about 2/27/2018 at 2350 Maitlin Way, Lot 20, Buford, GA a worker was observed performing caulk application activities from the roof where the extension ladder used to access and egress the roof was not secured in place while used on dirt with the anti-slip feet not in use exposing the worker to a 25-foot fall hazard.
