29 CFR 1910.28(b)(1)(i): The employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that was 4 feet (1.2 m) or more above a lower level was protected from falling by one or more of the following:  Guardrail systems, safety net systems, or personal fall arrest systems:  A.  On or about March 6, 2018, Mixer Room:  Employees working from an elevated work platform 8 feet from the concrete floor below, were not protected from falls by a guardrail system, safety net system or personal fall arrest system when working on the mezzanine:  1.  A 5-foot wide opening at the front of the mezzanine which was used for loading was not guarded.  2.  A 3-foot six-inch wide opening was unguarded at the back of the mezzanine where employees would gain access from the fixed ladder at Tank T3.

On or about March 6, 2018, Mixer Room:  Guardrails on the mezzanine were not provided with intermediate rails to prevent employees from falling 8 feet to the concrete floor below.  Openings in the guards exceeded 19 inches.

29 CFR 1910.151(c): Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use:   A.  On or about March 6, 2018, Facility:  Corrosive and caustic materials used to clean mixer tanks was kept in a locked cage in the shop near the manual fill line.  No eyewash was available in the immediate area.

On or about March 6, 2018, Bib Line:  An in-going nip point created by the 12-inch wide conveyor belt over a 6-inch diameter drive roller at the strapping end of the conveyor was unguarded, exposing employees working in the area to amputation hazards.

29 CFR 1910.303(b)(7)(iv): Electrical equipment had damaged parts that may adversely affect safe operation or mechanical strength of the equipment, such as parts that are broken, bent, cut, or deteriorated by corrosion, chemical action, or overheating:   A.  On or about March 6, 2017, Between Warehouse Bay 5 and Bay 6:  A 2-receptacle outlet box mounted to the wall between the bay doors just above floor level had been damaged and the outlets within the box  were loose.  Voltage was detected in the metal-encased Fostoria dock light powered through the upper receptacle of the outlet box.
