29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  a. On or about 02/02/2018, at building 10 - employees of Anaya Construction Services, Inc. were exposed to a 32 foot and a 9 foot fall hazard while standing on top of the walls during framing work, in that, fall protection was not used.

29 CFR 1926.852(a): Material was dropped to points lying outside the exterior walls of the structure which had not been effectively protected:  a. On or about 02/02/2018, at building 10 - employees were exposed to a struck-by hazard, in that, employees of Anaya Construction Services, Inc. were allowed to drop construction material debris from an elevated level of approximately 32 feet to the ground in areas where employees were permitted to walk. The work area below was not effectively protected.
