The most recent occurrence of this was evident during the inspection of the facility located at 8625 Military Road, Omaha, NE where the employer has employees operating and servicing equipment while being exposed to amputation hazards. Instances include but are not limited to: i. On the mixer the guard around the belt and pulley assembly fails to protect employees from reaching up under the guard during operation. ii. On the roller mill the guard around the belt and pulley assemblies on both sides fails to protect employees from the in going nip points.

29 CFR 1910.219(e)(1)(i): Horizontal belts which had both runs seven feet or less from the floor level were not guarded with a guard that extended to at least fifteen inches above the belt: The employer is failing to protect employees from caught between hazards associated with inadequately guarded belt assemblies. The most recent occurrence of this was evident during the inspection of the facility located at 8625 Military Road, Omaha, NE where the employer has employees operating and servicing equipment while being exposed to amputation hazards. Instances include but are not limited to: i. On the mixer the guard around the belt and pulley assembly fails to protect employees from reaching up under the guard during operation. ii. On the roller mill the guard around the belt and pulley assemblies on both sides fails to protect employees from the in going nip points.
