29 CFR 1910.22(a)(2): The floor of each workroom was not maintained in a clean and, to the extent feasible, in a dry condition.         On or about February 23, 2018, at the jobsite located 1616 Dielman Road, Saint Louis, Missouri the employer did not ensure the floor of each workroom was maintained in a dry condition in the following instances:         1. In the east corner of the showroom, the floor was wet resulting from a leaking roof that had not been repaired after several months of damage.  This left employee(s) exposed to a slip and fall hazard.       2. Standing water near the north wall of the warehouse from the leaking roof exposed employee(s) to a slip and fall hazard.       3. Standing water in the forklift battery changing area near the south wall of the warehouse potentially exposing employee(s) to both electrical and slip and fall hazard.

Protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, shall be provided, used, and maintained in a sanitary and reliable condition wherever it is necessary by reason of hazards of processes or environment, chemical hazards, radiological hazards, or mechanical irritants encountered in a manner capable of causing injury or impairment in the function of any part of the body through absorption, inhalation or physical contact.       On or about February 23, 2018, at the jobsite located 1616 Dielman Road, Saint Louis,  Missouri the employer did not employer did not provide personal protective equipment for employee(s) required to maintain powered industrial trucks in the following instances:         1. Employee(s) required to charge batteries in the battery charging area are not provided a face shield and apron to prevent contact with corrosive chemicals per manufacturer specifications.       2. Employee(s) required to change liquid propane cylinders are not provided gloves or eye protection to prevent contact with pressurized liquid propane.

On or about February 23, 2018, at the jobsite located 1616 Dielman Road, Saint Louis, Missouri, the employer did not ensure powered industrial trucks in need of repair, defective, or in any way unsafe was taken out of service in the following instances:       1.  The Yale Veracitor 40VX Forklift (Model No.  GLC0405VXNUSE 082; Serial No.  C809V09791E) was in operation with disconnected front lights, broken (inoperable) rear lights       2. The Nissan Forklift (Model No.  MCPL01A15LV; Chassis No.  CPL01 9N1313) broken rear lights with shrink wrap used to keep them in place.

Defects when found were not immediately reported and corrected.     On or about February 23, 2018, at the jobsite located 1616 Dielman Road, Saint Louis, Missouri, the employer did not ensure that when defects were found with Yale Veracitor 40VX Forklift (Model No.  GLC0405VXNUSE 082; Serial No.  C809V09791E); and Nissan Forklift (Model No.  MCPL01A15LV; Chassis No.  CPL01 9N1313) that they were either removed from service.  Warehouse employee(s) were observed operating the forklifts with disconnected headlights and broken, inoperable rear lights.
