29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, and rotating parts:    a) Plant - On or about 03/06/18 and at times prior thereto employees operated the ALS 4.0 automatic linear saw and the pin for the interlock (controlling the door and the brake) was not attached to the door, sending a false signal that the door was closed. The interlock was bypassed and the saw continued in operation when the door was opened.       Or in the Alternative:    29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s):    a) Plant - On or about 03/06/18 and at times prior thereto employees operated the ALS 4.0 automatic linear saw and the pin for the interlock (controlling the door and the brake) was not attached to the door, sending a false signal that the door was closed. The interlock was bypassed and the saw continued in operation when the door was opened.       In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR 1910.147(c)(6)(ii): The employer did not certify that periodic inspections of the energy control procedures had been performed:    On or about 03/06/18 and at times prior thereto employees performed maintenance/repairs and applied lockout/tagout on equipment including (but not limited to) the Alpine ALS 4.0 automatic linear saw., the Alpine ALS 276 automatic saw, the Automill, and the Alpine truss presses. The employer did not certify that periodic, and at least yearly inspections/audits of the specific written lockout/tagout procedures hd been performed.
