1926.102(a)(1): Employees were not provided with eye and face protection equipment when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:  (a) New residential construction located on Lot #35, Roof: On or about 02/21/2018, and at times prior, the employer exposed employees to struck-by hazards while operating pneumatic roofing nail guns at 120 PSI, in that protective eyewear was not provided to employees engaged in shingle installation.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m)  or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501 (b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502:   (a) New residential construction located on Lot #35, Roof: On or about 02/21/2018, and at times prior, the employer exposed employees to fall hazards while in stalling shingles on a 8/12 pitched roof at eave heights of 23-feet, 3-inches, in that a fall protection system was not provided or used while engaged in residential roofing activities.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  (a) New residential construction located on Lot #35, Front side: On or about 02/21/2018, and at times prior, the employer exposed employees to falls of 23-feet, 3-inches to the lower level, in that the ladder used to access the lower level was not extended at least 3-feet past the landing area to allow for safe access on and off the roof. The ladder used was extended 12-inches past the eave and not secured to the structure to prevent movement during use.
