29 CFR 1910.95(g)(1): The employer did not establish and maintain an audiometric testing program as provided by 29 CFR 1910.95(g) by making audiometric testing available to all employees whose exposures equal or exceed an 8-hour time-weighted average of 85 decibels: FACILITY On, or about, March 06, 2018, the employer failed to perform annual audiometric testing for all employees whose noise exposure exceeded an 8-hour time-weighted average of 85 decibels. On March 15, 2018, an employee operating an inline thermoformer was exposed to noise levels of 87.00 dBA when monitored for a period of 467 minutes. Zero exposure was assumed for the 13 minutes not monitored.
