29 CFR 1910.95(d)(1): When information indicated that any employee's exposure equaled or exceed the 8-hour time-weighted average of 85 decibels, the employer did not develop and implement a monitoring program:    (a)    Handle Mill:  On or about March 6, 2018 the employer had not performed noise monitoring where employees were required to wear hearing protection in that an employee (Chop Saw Operator) was exposed to continuous noise at 281.7 percent of the permissible daily noise exposure (8-hour Time Weighted Average sound level of 90 dBA) during the 480 minute sampling period on March 13, 2018.  For a full eight hour work shift this exposure would be equivalent to an average sound level of 97.4 dBA.    (b)    Handle Mill:  On or about March 6, 2018 the employer had not performed noise monitoring where employees were required to wear hearing protection in that an employee (Rip Saw Operator) was exposed to continuous noise at 164.7 percent of the permissible daily noise exposure (8-hour Time Weighted Average sound level of 90 dBA) during the 480 minute sampling period on March 13, 2018.  For a full eight hour work shift this exposure would be equivalent to an average sound level of 93.5 dBA.    (c)    Handle Mill:  On or about March 6, 2018 the employer had not performed noise monitoring where employees were required to wear hearing protection in that an employee (Dowel Milling Machine Operator) was exposed to continuous noise at 110.9 percent of the permissible daily noise exposure (8-hour Time Weighted Average sound level of 90 dBA) during the 480 minute sampling period on March 13, 2018.  For a full eight hour work shift this exposure would be equivalent to an average sound level of 90.7 dBA.

29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:    (a)  Handle Mill:  On or about March 6, 2018 the employer had not developed and documented energy control procedures for equipment on site including, but not limited to, the L-M Pond and Deck Saw Model 75 HC Serial Number 1765 that outlined the scope, purpose, authorization, rules and techniques to control hazardous energy and the means to enforce compliance with the procedures.  This equipment has hydraulics and electric as its energy sources.    (b)  Handle Mill:  On or about March 6, 2018 the employer had not developed and documented energy control procedures for equipment on site including, but not limited to, the Log Carriage that outlined the scope, purpose, authorization, rules and techniques to control hazardous energy and the means to enforce compliance with the procedures.  This equipment has electric and air as its energy sources.    (c)  Handle Mill:  On or about March 6, 2018 the employer had not developed and documented energy control procedures for equipment on site including, but not limited to, the Head Saw that outlined the scope, purpose, authorization, rules and techniques to control hazardous energy and the means to enforce compliance with the procedures.  This equipment has electric and air as its energy sources.    (d)  Handle Mill:  On or about March 6, 2018 the employer had not developed and documented energy control procedures for equipment on site including, but not limited to, the electric operated Dowel Milling Machine that outlined the scope, purpose, authorization, rules and techniques to control hazardous energy and the means to enforce compliance with the procedures.  This operation has had a history of serious injuries related to improper   energy isolation.

29 CFR 1910.95(g)(7)(i): Each employee's annual audiogram was not compared to that employee's baseline audiogram to determine if a standard threshold shift as defined in 29 CFR 1910.95(g)(10) has occurred:    (a) Handle Mill:  On or about March 6, 2018 the employer had not compared an employee's annual audiogram to that employee's baseline audiogram to determine if a standard threshold shift had occurred in that the employee (Chop Saw Operator) experienced a standard threshold shift after his December 2017 audiogram.  The employee was exposed to continuous noise at 281.7 percent of the permissible daily noise exposure (8-hour Time Weighted Average sound level of 90 dBA) during the 480 minute sampling period on March 13, 2018.  For a full eight hour work shift this exposure would be equivalent to an average sound level of 97.4 dBA.    The Collins Tool Corporation was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.95(g)(7)(i), which was contained in OSHA inspection number 924665, citation number 1, item number 3A and was affirmed as a final order on January 2, 2014, with respect to a workplace located at 3254 Old Stage Road, Lewistown, PA 17044.
