29 CFR 1926.404(f)(6): The path to ground from circuits and equipment was not permanent and continuous: a) During construction renovation of 3 story building, employees were exposed to electric shock hazard using ungrounded electric equipment plugged into ungrounded power outlets; the outlets were hanging by their wires from the walls, attached by wire nuts with no tape and no ground connections. 29 CFR 1926.451(h)(2): Where there is the danger of tools, materials, or equipment falling from a scaffold and striking employees below, the employer did not provide preventive measures; to include: i) Barricade the area below the scaffold; or ii) Erect toeboards on the scaffold; iv) Install a guardrail system with openings small enough to prevent passage of falling objects; v) Erect a canopy structure, debris net, or catch platform above employees working below the scaffold. a) There was no overhead protection at the front entrance to the building underneath the scaffolding; employees using the entrance are exposed to falling tools and masonry materials. No toeboards, canopy or other overhead protection was provided. Location: 1644 58th Street, Brooklyn N.Y.
