29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.        a) Second Floor: Employees pouring concrete were exposed to a fall hazard of approximately 14' fall from an unprotected side, on or about 3/05/2018.    b) First Floor/Elevator Shaft: Employees pouring concrete were exposed to a fall hazard of approximately 24' to the bottom of an unprotected elevator shaft from a second floor, on or about 3/05/2018.

29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights), more than six feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes.      a) Second Floor: Employees pouring concrete were exposed to a fall hazard of approximately 14' from an unprotected hole on a floor, on or about 3/05/2018.    b) First Floor/Elevator Shaft: Employees pouring concrete were exposed to a fall hazard of approximately 10' from an unprotected elevator shaft opening on the first floor, on or about 3/05/2018.
