(i) the employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that is 4 feet (1.2 m) or more above a lower level is protected from falling:        a. On March 7, 2018, in the tree, an employee cutting tree limbs was not protected from a 14 feet fall hazard by the use of a fall protection system.

On March 7, 2018, in the tree and on the ground, employees using chain saws to cut tree limbs were not wearing safety glasses, exposing themselves to an eye hazard.
