29 CFR 1926.451(g)(1)(vii): Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section.    Employees were exposed to falling 18 ft from the pipe frame scaffolds during construction renovation of the 3 story building:      a)  Guardrails were not provided at the ends of the scaffold platforms and where the platforms extend beyond the corner of the building.       Location: 1644 58th Street, Brooklyn N.Y.      On or about 2/13/2018

29 CFR 1926.451(h)(2): Employees working below a scaffold were not protected from falling tools, materials or equipment in accordance with the provisions of paragraphs (h)(2)(i)-(v) of this section:      a)  Overhead protection (canopy or equivalent) was not provided below the pipe frame scaffold at the front of the building during masonry stucco operations overhead on the scaffold; employees going in or out of the building were exposed to falling objects hazard (masonry tools, cement).      Location: 1644 58th Street, Brooklyn N.Y.      On or about 2/13/2018      NOTE: THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM IN ACCORDANCE WITH 29 CFR 1903.19.
