29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  a)  At Lot 32 Boykin Lakes Loop, Pike Road, Alabama:  On or about March 7, 2018 and at times prior; the employer allowed his employees to install siding while standing on a roof without fall protection exposing them to a fall hazard of 11 feet to the ground below.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  a)  At Lot 32 Boykin Lakes Loop, Pike Road, Alabama:  On or about March 7, 2018 and at times prior; the employer allowed his employees to install siding while standing on a roof without first ensuring his employee was trained on the hazards of falls to include but not limited to setting up fall protection and inspecting fall protection exposing his employees to a fall hazard of 11 feet to the ground below.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  a)  At Lot 32 Boykin Lakes Loop, Pike Road, Alabama:  On or about March 7, 2018 and at times prior; the employer allowed his employees to use an extension ladder to gain access to the roof top and the rails of the ladder did not extend at least three feet above the landing to provide safe access exposing his employees to a fall hazard of 11 feet to the ground below.
