29 CFR 1926.502(d)(21): Personal fall arrest systems were not inspected prior to each use for wear, damage, and other deterioration, and/or defective components were not removed from service. a) 801 Daylight Dr, Clay NY, On or about 3/8/18: Employees were using uninspected Personal Fall Arrest Systems, while installing roofing materials, that showed excessive wear, deterioration and damage that if properly inspected prior to use would not have been put into service. 29 CFR 1926.503(a)(2)(ii): The employer did not assure that each employee, as necessary, had been trained by a competent person in the correct procedures for erecting, maintaining, disassembling and inspecting the fall protection systems to be used. a) 801 Daylight Dr, Clay NY, On or about 3/8/18: Employees were using Personal Fall Arrest Equipment without being trained by a Competent Person in how to maintain and properly inspect the equipment prior to use.
