29 CFR 1910.28(b)(5)(i): The employer did not ensure each employee on a runway or similar walkway was protected from falling 4 feet (1.2 m) or more to a lower level by a guardrail system. On or about February 28, 2018, and at times prior there to, an employee was working from a catwalk platform that was not guarded with standard railings, nor was the employee provided with a means of fall protection. The employee was exposed to a fall hazard.

29 CFR 1910.212(a)(5): Fan blade guard(s) were not provided where the periphery of the blades was less than seven feet from the floor or working level: On or about February 28, 2018, and at times prior there to, an employee was performing unjamming duties on the Hytrol conveyor machine while standing on a conveyor line. The employee was exposed to struck-by hazards from fan blades that were not guarded.
