The employer did not assure that the energy control procedure for robotic cell 0214 and associated OHMA Clinch Nut machine was inspected.

OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees: O or about March 5, 2018, an employee of Respondent was exposed to injuries in the OHMA Clinch Nut machine in cell 0241 because the employer did not fully evaluate the robotic cell prior to placing it into production. The employer failed to ensure no program latches remained after installation and that the cell was functioning as intended prior to placing into production.

29 CFR 1910.147(d)(6): The employer did not ensure that prior to starting work on machines or equipment that have been locked out or tagged out, the authorized employee shall verify that isolation and deenergization of the machine or equipment have been accomplished: On or about March 5, 2018, the employee was exposed to injuries from pinch points in the OHMA Clinch Nut machine in cell 0241 because the employee did not verify that isolation and de-energization of the machine had been accomplished. The employer did not ensure that the employee verified isolation and de-energization of energy sources prior to starting work on tasks such as cleaning and unjamming the machine because the employer did not create and implement appropriate steps for the employee to follow.
