29 CFR 1926.102(a)(1): The employer shall ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation:   (a). Worksite located at 14831 E. Hwy 40, Kansas City, Missouri- Employees were observed using pneumatic nail guns while installing roof shingling and were not protected from eye injuries by the use of safety glasses.

29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:   (a). Worksite located at 14831 E. Hwy 40, Kansas City, Missouri- Employees were engaged in residential roofing work and were not protected from falling by the use of any means of fall protection.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:   (a). Worksite located at 14831 E. Hwy 40, Independence, Missouri- Employees were exposed to a fall hazard in that employees were using an extension ladder that was not extended at least 3' above the upper landing surface.
