29 CFR 1910.147(a)(2)(ii)(B): Lockout/tag out procedures were not implemented when servicing and/or maintenance was taking place during normal production operations when an employee was required to place any part of his/her body into area on a machine or piece of equipment's point of operation: The Side Matcher Machine Area of the facility - On February 28, 2018, an employee was exposed to serious injuries to the hands and or fingers while engaged in set-up activities on the Hasko Side Matcher Machine, Serial #0443704. The employee placed his hand into the Point of Operation of the live machine where none of the energy sources had been locked/tagged out and or blocked out. As a result, the employee's hand was crushed which lead to an amputation of one finger and serious injuries to the others.

29 CFR 1910.147(c)(4)(ii)(B): The energy control procedure did not clearly and specifically specific procedural steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy: The Side Matcher Machine Area of the facility - On February 28, 2018, the employer had not developed adequate machine specific procedures for the Side Matcher Machine SN#0443704, to include but not limited to lock-out/tag-out, and or blocking and securing to control the hazardous energy of the machine. As a result, an employee's hand was crushed in the danger zone of the machine, which led to an amputation of one finger and serious injuries to the others.
