29 CFR 1926.453(b)(2)(iv): Employee(s) working in an aerial lift were not standing firmly on the floor of the basket: On or about March 7, 2018, an employee stood on the mid-rails of a 60ft boom JLG lift basket to install sheet rocks at about 36ft above the ground on the third floor of the west side of the building. This condition exposed the employee to fall from height hazards.

29 CFR 1926.453(b)(2)(v): A lanyard was not attached to the boom or basket when working from an aerial lift: On or about March 7, 2018, an employee did not attach a lanyard to the basket when exiting and reentering a 60ft boom JLG lift basket at approximately 36ft above the ground on the third floor of the west side of the building. This condition exposed the employee to fall from height hazards.
