29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  a) MATES Building, Fort Pickett, Blackstone, VA 23824: On March 1, 2018, and times prior thereto, the employer did not guard the turbo air inlet fan blade during ground hop engine maintenance activities on the M109 Palladin tank, exposing employees to caught-in hazards.  Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.

29 CFR 1904.39(a)(2): The employer did not report an in-patient hospitalization, amputation, or loss of an eye as a result of a work-related incident to OSHA within twenty-four (24) hours.  a) MATES Building, Fort Pickett, Blackstone, VA 23824: On March 1, 2018, at approximately 2:00 p.m., a mechanic suffered an amputation of the first and middle fingers that resulted in an in-patient hospitalization. The employer did not report the amputation to the Occupational Safety & Health Administration until after the 24-hour reporting period.  No abatement documentation or certification required.

29 CFR 1904.41(a)(1): Annual electronic submission of part 1904 records by establishments with 250 or more employees. The establishment had 250 or more employees at some time during the previous calendar year, and this part required the establishment to keep records, but the employer did not electronically submit information from OSHA Form 300A Summary of Work-Related Injuries and Illnesses to OSHA or OSHA's designee. The employer did not submit the information once a year, no later than the date listed in paragraph (c) of this section of the year after the calendar year covered by the form:  a) MATES Building, Fort Pickett, Blackstone, VA 23824: On March 8, 2018, and times prior thereto, the employer failed during calendar year 2017 to electronically submit information from the OSHA Form 300A or equivalent by December 15, 2017. The establishment employed approximately 522 employees and was classified in 928110 NAICS during calendar year 2016.  No abatement documentation or certification is required.
