OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to amputation hazards:  (a)Pressing area: Chicago Mechanical press brake: On or about March 6, 2018 and at times prior thereto the employer exposed his employees to crush-by and amputation hazards in that employees were allowed to use a foot pedal that was not equipped with a protective cover to operate the Chicago mechanical press brake.  (b)Pressing area: Cincinnati 3 Mechanical press brake: On or about March 6, 2018 and at times prior thereto the employer exposed his employees to crush-by and amputation hazards in that employees were allowed to use a foot pedal that was not equipped with a protective cover to operate the Cincinnati 3 mechanical press brake.
