29 CFR 1926.20(b)(2): The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person(s).  On or about March 6, 2018, workers were conducting framing operations on the 13 acre, multi-family apartment complex without frequent and regular safety inspections being conducted on the site where serious hazards exist, exposing workers to hazards such as, but not limited to falls, inhalation of crystalline silica, struck by flying objects and stepping on protruding nails.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  On or about March 6, 2018, eight workers were installing roof trusses and decking materials on the third story roofs of Buildings 4 & 5 up to approximately 34 feet high without fall protection, exposing the workers to a fall hazard.

29 CFR 1926.501(b)(14): Each employee working on, at, above, or near wall openings (including those with chutes attached), where the outside bottom edge of the wall opening was 6 feet (1.8 m) or more above lower levels and the inside bottom edge of the wall opening is less than 39 inches (1.0 M) above the walking/working surface, is not protected from falling by the use of a guardrail system, a safety net system, or a personal fall arrest system.  On or about March 6, 2018, workers were walking and working around wall and balcony openings on the second and third floors of buildings 4 & 5 without protective railing in place, exposing the workers to fall heights up to approximately 20 feet.

29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights), more than six feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes.  On or about March 6, 2018, employees were accessing the second and third floor levels of buildings 4 & 5 under construction where ladder openings through the floors were not protected with guard railing, exposing employees to a fall hazard up to approximately 10 feet.
