Employees were exposed to a fall hazard of approximately eight (8) feet during walking and/or work operations in this area.

NAC Carbon Products, Inc., on or about March 9th, 2018, and times prior thereto: Employees were required to wear loose-fitting, powered air purifying respirators (Versaflow brand #TR-300-LIK) during shoveling operations of acetylene black and petroleum coke in the graphitizing areas of the facility. Employees were not provided with a medical evaluation prior to the use of the respirators onsite.

NAC Carbon Products, Inc., on or about April 11th, 2018, and times prior thereto: A graphite room operator/laborer performing work operations including, but not limited to, shoveling operations of carbon black (Soltex brand acetylene black) in the graphite room areas was exposed to carbon black at a Time-Weighted Average of 4.82 mg/m3. The exposure level was derived from a sample collected over a 424 minute period; the calculation included a zero value for the 56 minutes not sampled.
