29 CFR 1926.1101(f)(1)(i):  Where exposure monitoring was required under this section, the employer did not perform monitoring to determine accurately the airborne concentrations of asbestos to which employees were or potentially were exposed:       (a) CTL-Thompson at 1777 6th Street in Boulder, CO 80302:  On or before February 22, 2018, the employer had workers working around and within an asbestos containment during removal of 2% chrysotile material.  The employer did not perform personal air monitoring on employees to determine their exposure.

29 CFR 1910.1200(e)(1):  The employer had not developed or implemented a written hazard communication program included the requirements outlined in 29 CFR 1910.1200(e)(1)(i) and (e)(1)(ii):      (a) CTL-Thompson at 1777 6th Street in Boulder, CO 80302:  On or before February 22, 2018, the employer did not develop and implement a written hazard communication program that pertains to employee use of chemicals such as, but not limited to the following: acetone and triacetin.  This condition exposed employees to chemical exposure hazards. 29 CFR 1910.134(c)(1): The employer did not establish and maintain a written respiratory protection program, as specified in subparagraphs (c)(1)(i) through (ix), in that employees were required to wear respirators during work operations:     (a) CTL-Thompson at 1777 6th Street in Boulder, CO 80302:  On or before February 22, 2018, the employer did not have a written respiratory protection program with worksite specific procedures, as specified in subparagraphs (c)(1)(i) through (ix) of this section. The program was not established and implemented whenever respirators were required by the employer, in that the employer required the use of air filtering respirators during asbestos abatement project oversight. This hazard exposed the employee to hazardous chemicals due to the lack of knowledge of proper respirator use.     Note:  29 CFR 1926.103: The requirements applicable to construction work under this section are identical to those set forth at 29 CFR 1910.134 of this chapter.

29 CFR 1910.134(f)(5):  The employer did not ensure that the fit test was administered using an OSHA-accepted QLFT or QNFT protocol:     (a) CTL-Thompson at 1777 6th Street in Boulder, CO 80302:  On or before February 22, 2018, the employer did not perform an effective fit test for each respirator; during qualitative fit testing, the test subjects were not required to perform the full fit testing requirements, including deep breathing, bending, jogging in place and the second set of normal breathing exercises. The documentation of the fit test data did not note respirator size worn during the test. This hazard exposed the employee to hazardous chemicals due to the lack of knowledge of proper respirator use.    Note:  29 CFR 1926.103: The requirements applicable to construction work under this section are identical to those set forth at 29 CFR 1910.134 of this chapter.

29 CFR 1910.134(k): Training and information. This paragraph requires the employer to provide effective training to employees who are required to use respirators. The training must be comprehensive, understandable, and recur annually, and more often if necessary. This paragraph also requires the employer to provide the basic information on respirators in Appendix D of this section to employees who wear respirators when not required by this section or by the employer to do so.   (a) CTL-Thompson at 1777 6th Street in Boulder, CO 80302:  On or before February 22, 2018, the employer did not provide employees with effective information and training on respiratory protection to wear in their work area at the time of their initial assignment, and at least annually thereafter.  This condition exposed employees to respiratory hazards.
