29 CFR 1926.404(b)(1)(i): Employer did not use either ground fault circuit interrupters as specified in paragraph (b)(1)(ii) of this section, or an assured equipment grounding conductor program as specified in paragraph (b)(1)(iii) of this section to protect employees on construction sites.  Material staging area: A Leviton ground-fault circuit-interrupter receptacle used to supply power to a Hilti UD 30 corded drill was defective, and there was no assured equipment grounding conductor program to protect the employee who used the drill.  Employees were exposed to electrical hazards.

29 CFR 1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws:  Back Side:  The outer insulation of the power cord affixed to a flood lamp was pulled away from the junction box.  Employees were exposed to electrical and fire hazards.
