29 CFR 1910.147(c)(4)(i): Procedures were not utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:   a. 356 Meredith Ave., Staten Island, NY  The employer did not ensure equipment specific procedures developed for hazardous energy control were utilized by employees.  Employees engaged in the cleaning of asphalt manufacturing equipment conveyor rollers did not utilize LOTO procedures on the equipment when cleaning.  An employee received amputation injuries when cleaning the belt rollers while the equipment was being operated.  Violation occurred on or about March 12, 2018  b. 356 Meredith Ave., Staten Island, NY  The employer did not ensure equipment specific procedures developed for hazardous energy control were utilized by employees.  Employees engaged in the maintenance and repair of asphalt manufacturing equipment gear and chain drive mechanisms on top of the silo did not utilize LOTO procedures on the equipment prior to beginning their work activities.  Employees performing repairs to a silo chain and sprocket performed maintenance on equipment without specific procedures to ensure that the equipment could not be engaged.  Violation occurred on or about March 12, 2018

Locks and tags were not applied to the circuit breakers or the circuit breaker cabinet indicating which energy sources were deactivated while employees performed repairs at the top of the silo.
