29 CFR 1926.403(b)(2): Listed, labeled, or certified equipment was not installed and used in accordance with instructions included in the listing, labeling, or certification: A) 1012 Hanover St Piscataway, NJ 08854 Instance A: More than one ungrounded (Hot) conductor was terminated at the line side terminal lug within the fused disconnect switch enclosure. Instance B: The grounded (Neutral) and grounding (Bare) were both terminated at the upturned lug on the neutral busbar. Instance C: The 12 AWG Non-metallic conductor (yellow Romex) is not permitted to be installed in Wet or Damp locations. Instance D: The relocatable power tap was modified in that the attachment plug was removed and spliced directly into the Non-metallic conductor. Instance E: The relocatable power tap was used in an outdoor environment, when they are only listed and labeled for indoor use.
