29 CFR 1910.178(m)(5)(iii): When the operator of an industrial truck is dismounted and within 25 ft. of the truck still in his view, the load engaging means were fully lowered, the controls were not neutralized, and the brakes were not set to prevent movement. Site: 1941 Broadway Brooklyn, NY On or about 3/10/18 a) An employee operating a forklift, dismounted the truck and the brakes were not engaged to prevent movement. The forklift rolled and pinned the employee between the forklift and the flatbed truck.
