29 CFR 1910.134(c)(2)(i): Respirator users were not provided with the information contained in Appendix D to 29 CFR 1910.134 when the employer determined that any voluntary respirator use was permissible: (Construction Reference 1926.103): On or about March 12, 2018, employees were allowed to wear respirators in the workplace without being provided the information contained in appendix D, exposing employees to the hazards associated with respirator use. JACK LEE CONSTRUCTION SERVICES, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.134(c)(2)(i) which was contained in OSHA inspection number 1286927, citation number 2, item number 1 and was affirmed as a final order on February 27, 2018, with respect to a workplace located at 5135 Main St. Downers Grove, IL 60515. Abatement certification is required of this item in accordance with the requirements of 29 CFR 1903.19(c).
