OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to struck-by and crush-by hazards:  On or about 3/8/2018 at 2750 Breckenridge Boulevard, #400 Duluth, GA employees on foot pulling product or walking by the racks with visibly damaged stretchers and uprights were exposed to broken bones and lacerations where powered industrial trucks could make contact with the damaged racks causing collapse or displacement, locations include but are not limited to:  a. Rack 1, b. Rack 2  In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR 1910.133(a)(1): The employer did not ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation:  On or about 3/8/2018 at 2750 Breckenridge Boulevard, #400 Duluth, GA operators performing propane tank changes on the powered industrial trucks were exposed to eye injuries where proper eye protection was not offered by the employer. Operators changing out the propane wore eyewear including but not limited to non-chemical rated safety glasses and prescription.
