29 CFR 1910.28(b)(1)(i): (1) Unprotected sides and edges. (i) Except as provided elsewhere in this section, the employer must ensure that each employee on a walking-working surface with an unprotected side or edge that is 4 feet (1.2 m) or more above a lower level is protected from falling by one or more of the following: a. On or about January 26, 2018, at 1351 Old Hwy 51 Arbor Vitae, WI, conventional fall protection was not utilized while an employee was removing snow from a residential roof. The employee fell from the roof approximately four feet to the snow which had an approximately nine foot eve height without snow. Conventional fall protection includes: 1. Guardrail systems 2. Safety net systems 3. Personal fall protection systems, such as personal fall arrest, travel restraint, or positioning systems.

29 CFR 1910.30(a)(1): (a) Fall hazards. (1) Before any employee is exposed to a fall hazard, the employer must provide training for each employee who uses personal fall protection systems or who is required to be trained as specified elsewhere in this subpart. Employers must ensure employees are trained in the requirements of this paragraph on or before May 17, 2017. a. On or about January 26, 2018, the employer located at 10816 Townline Rd Woodruff, WI 54568 did not provide training regarding personal fall protection systems to an employee who was required to use a personal fall protection system when removing snow from the roof of a residence located at 1351 Old Hwy 51 Arbor Vitae, WI. The eave height of the residence was approximately eight feet
